As a full service DME provider, we coordinate all information between physician, therapist, patient and insurance company. If you are a clinician interested in providing an instrument to your patient and would like to check on insurance simply fill out the Patient Information Sheet and we will contact the patients insurance and have an answer within one business day.
Otherwise, to arrange a home rental unit:

Fill out the paperwork with Pt. signature, where needed, and dispense unit to patient.

Therapists are responsible for mailing or faxing paperwork back to CMT.

Current Medical Technologies will bill insurance companies or Patients directly as required.
